Once completed you can upload the document to our contact form below.
Your dentist will need to fill out the following form and ensure they attach the relevant X-rays, a medical history form, MDMAS (dental anxiety form) and provide your height and your weight.
Any information missing with result in a rejected referral and delay in your appointment.
